Appointments

To schedule an appointment at Huntsville Reproductive Medicine, please contact our office at 256-213-BABY (2229) between the hours of 8:00 am and 5:00 pm. We will call you a few days before your initial visit to confirm your appointment. If you will not be reachable by phone for this confirmation, please contact us by another method so that we can keep the appointment slot for you. If you must cancel or reschedule your appointment, please give a minimum of 24 hours notice.

When we make your initial appointment, we will need to obtain some insurance information from you. Please have your insurance card and policy number available so that we can verify your coverage in advance. We will also request your e-mail address so that we can e-mail you a new patient history form. Please take the time to complete the form in its entirety and bring it to your appointment.

We will also need records from any previous infertility evaluation that you may have had. If you are already in possession of your old medical records, you will need to bring them to your first visit. If your medical records are still at the previous doctor's office, we will need to obtain your signature (either in person or by fax) so that we can lawfully request copies of those records.
